I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the event, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with the game and in consideration for the (Be Limitless FS) accepting the registrant for its Futsal/Beach/Outdoor soccer programs and activities, I hereby release, discharge, and/or indemnify the (Be Limitless FS), its affiliated organizations and sponsors, their employees and associated personnel, including the owners of gymnasiums and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the registrant's participation in the Programs, I hereby authorize.

REFUND POLICY: Full refunds, will be provided if request is submitted 7 days prior to camp start, $10 administration fee per camp will be deducted from all participant refund request. Partial refund will be provided if request is submitted within the 7 day period.
